Had a local glass shop cut these for me. Tried about 10 before I found one that fit correctly. Place the glass in the rectangular slot of the lens assy formerly occupied by the IR filter. Place the gasket over the glass piece so that it will contact the CCD when you re-attach the lens assembly. Make sure you thoroughly clean the replacement glass and use tweezers to place.
